Transitions, Planning & Monitoring (TPM) Software is our core software solution currently being implemented within a variety of schools and school divisions / districts across Canada.   TPM is a web based application that has been custom developed by our Leadership Services Team.  Currently, TPM supports the collection and reporting of various data variables through either standardized forms (i.e. Individualized Education Plans) or customized templates (i.e. client surveys, special education case management meetings, grade to grade transition templates, school based intervention tracking systems, specialized equipment and training logs, etc.).

Current TPM implementation include:

• Individual Program Plans, clinician case note management and various educational checklists including Academic Accommodations checklist, General Accommodations checklist etc. within Alberta schools / divisions
• Individual Education Plans, Adaptive Learning Plans, Grade to Grade (i.e. Gr 5 – Gr. 6) transition templates, Student Support Team management templates, clinician case management templates within Manitoba
• Individual Education Plans, Grade to Grade (i.e. Gr 8 – Gr. 9) transition templates, Student Success team intervention tracking and case load management templates, SEA equipment tracking, Specialized Alternative Learning Plan (SALP) templates and communication logging, staff attendance tracking and school profiles within Ontario School Districts
